1             INTRODUCTION

 

COVID-19 Project

 

The social and economic impacts of the COVID-19 could be severe.  The external impact from Covid-19 and the authorities’ measures to prevent the outbreak are likely to have a significant negative impact on the Mongolian economy and thus on poverty reduction, education and health outcomes. The COVID-19 Project shall be implemented over a three-year period. The original Project effectiveness date was April 24, 2020, and the additional financing was signed on 15 February 2021 and ratified on 2 April 2021. The purpose of the Additional Financing is to help the Government of Mongolia (GOM) to purchase and deploy COVID-19 vaccines that meet Bank standards.

 

The scope of this project will be nationwide, benefiting the entire population of 3.2 million of which 1.5 million (47%) live in Ulaanbaatar, the capital city. The project interventions will cover all 21 Aimags and their soums, and selected hospitals. The primary beneficiaries of the Project will include the patients visiting hospitals and health facilities. Patients will receive timely and comprehensive diagnostic and treatment services and care will be managed effectively and efficiently through identified care pathways. The second group of beneficiaries will include the community at large, especially vulnerable and high-risk populations such as the elderly. Support for community outreach services will include advocacy on non-pharmaceutical public health measures (hand washing, respiratory etiquette, social distancing etc.) together with risk communication among the general population which will help reduce number of at-risk and infected patients. The third group of beneficiaries will include health care providers in Aimags, Districts and selected hospitals who will be providing care to COVID-19 infected and other patients. People who is over 18 years old will be vaccinated voluntary. Health care workers and the target population shall be prioritized in case of limited availability of the vaccine, and vaccination shall be carried out systematically in phases.

 

Public Health Specialist requirements

 

The primary objectives of this AF are to enable affordable and equitable access to COVID-19 vaccines, to help ensure effective vaccine deployment in Mongolia through enhanced vaccination system and to further strengthen preparedness and response activities under the parent project. The Project shall strengthen Mongolia’s capacity to prevent and respond to the COVID–19 pandemic and strengthen national health systems for public health preparedness. The PDO will remain unchanged for the AF. The parent project comprises of four components: (i) Emergency COVID-19 Prevention and Response; (ii) Strengthening Health Care Delivery Capacity; (iii) Implementation Management and Monitoring and Evaluation; and (iv) Contingent Emergency Response Component. 

 

Public health specialist to provide and manage public health activities based on existing UNICEF communication strategy, a plan for risk communication will be prepared, and key messages for COVID-19 vaccination advocacy and communication developed with special focus on vaccine hesitancy. A plan for immunization services for specific target groups has been designed; permanent, temporary and mobile immunization points are determined, based on target population type. Adverse events following immunization AEFI surveillance need to be done by Public health specialist. Training to the medical staff on Post Vaccine Adverse reaction will be conducted.

 

2             OBJECTIVES

 

The consultant shall provide support in the all aspects of vaccine deployment related activities and ensure effective coordination with relevant stakeholders as required by MOH and the World Bank. And provide support in improving the public health activities under the strategic framework of the Mongolia COVID-19 Emergency Response and Health System Preparedness Project.

 

The local consultant shall undertake the following:

 

1.   Support and monitor the implementation of the vaccine deployment plan, Vaccine Delivery Distribution Manual, Stakeholder Engagement plan and Environmental and Social Management plan.

2.   Support the strengthening of the post-immunization adverse reaction reporting mechanisms.

3.   Support to an enabling environment for One Health program strengthening the capacity for multi-sectoral response operations to emerging and new infectious diseases.

4.   Cooperate with IPIU and implementing partners, participate as team member in other project activities.

 

3             SCOPE OF CONSULTING SERVICES

 

In order to support the effective project implementation, local consultant shall perform duties as follows:

 

3.1. Support and monitor the implementation of the Stakeholder Engagement plan, Environmental and Social Management plan and other Single source contracts with the NCZD, NCCD, National Center for Mental Health.

 

3.1.1   Provide support for enhancing cold chain.

3.1.2   Provide support for organizing a mobile vaccination unit.

3.1.3   Provide inputs for monitoring the performance of vaccine deployment, identify any bottlenecks to optimal performance and suggest solutions for improvement.

 

3.2    Support the creating an enabling environment for One Health.

3.2.1.   Support establishing a data exchange platform between the Ministry of Health, Ministry of Food and Light Industry and GAVS (General Authority for Veterinary Services) both for early warning systems and joint control of disease outbreaks.

3.2.2.   Support capacity for joint response to new and emerging infectious diseases.

3.2.3.   Organize Nationwide simulation exercise on One health together with the MOH, GAVS, MOFALI.

3.2.4.   Analyse and improve the collaboration between the MOH, GAVS, MOFALI for the prevention, detection and response to zoonotic diseases and other health events at the animal-human interface.

3.2.5.   Strengthen public health law enforcement and review to address inconsistencies on One Health.

3.2.6.   Conduct joint working group and implement multi stakeholders engaged activities on One Health.

3.2.7.   Support NCZD single source contract TOR development and monitor any One health related activities.

 

3.3.       Provide support in improving the capacity of the human resources in COVID-19 vaccination, post immunization adverse reaction, grievance mechanism.

 

3.3.1   Provide support to develop and the implementing process of the Aimag and district hospitals emergency and vaccination plan.

3.3.2   Provide support to develop and the implementing process of the Aimag and district hospitals emergency and vaccination plan.

3.3.3   Provide support in the development and dissemination of the public messages, training, and information materials aimed at improving knowledge on COVID-19 vaccination.

3.3.4   Strengthen COVID-19 vaccine related webpage through establishing and operationalizing the grievance mechanism that will receive, record, resolve and respond to concerns and complaints related to post COVID-19 vaccination adverse reaction from project affected parties.   

3.3.5   Strengthen the capacity and ensure sustainability of a grievance mechanism of the NCCD on COVID-19 vaccination specifically related to post immunization adverse reaction.

3.3.6   Supporting equity in vaccination (ex. ensuring vulnerable groups receive vaccination), boosters, and adverse reactions in collaboration with the Risk Communication and Social Specialist.

 

3.4.       Cooperate with IPIU and implementing partners, participate as team member in other project activities.

 

3.4.1.   Provide assistance in preparing the project implementation plan, submitting such documents to the related authorities for review and approval.

3.4.2.   Cooperate with IPIU, WHO and implementing partners, participate as team member in other project activities.

3.4.3.   Cooperate with an IPIU implementing Effective vaccine management and Rapid coverage assessment.

3.4.4.   Represent the PIU on the national committees and technical working groups and provide regular information sharing and project updates.

 

4             QUALIFICATIONS AND EXPERIENCE

 

·            Bachelor or higher degree (Master degree in relevant field is an advantage)

·            University graduate in public health or medical science

·            At least 3 years of relevant experience in the field of Public health (Experience in routine vaccination programs, outbreaks, and One health is an advantage);

·            Provide at least 2 reference letters to prove these required skills and on successful completion of similar scope and nature assignments from previous clients.

·            Strong quantitative analytical skills combined with a solid understanding of qualitative research methodologies;

·            High integrity and accountability, good team work spirit

·            Extensive computer skills

·            Highly developed analytic skills on conducting risk assessment, and public health research

·            Ability to prepare plans and write reports

·            Good written and spoken English (supported by certificates or other relevant documents) will be an advantage

·            Ability to work in rural areas for an extended period of time is an advantage.

5             INSTITUTIONAL ARRANGEMENT

 

The Public Health local consultant shall report to the IPIU Project Coordinator and the overall consultancy will be under the oversight of the Project Director. Daily supervision of the local consultant shall be done by IPIU Risk communication and community engagement specialist.

 

The Consultant will carry out the tasks described in this Terms of References (TOR) from November 2021 for 12 months continuously at the IPIU office in Ulaanbaatar, Mongolia. Occasional field trips to 21 Aimag and selected soums may be required.   

 

6             REPORTS AND OUTPUTS/DELIVERABLES

 

6.1.       Reports

 

The Consultant shall furnish the following reports:

 

  • Quarterly reports by 10th day of the next quarter’s first month, summarizing achievements made, deliverables submitted and other relevant significant information, “Consultant acceptance report” must be completed and attached to the monthly report,
  • Final report upon completion of the consultancy. The report should include a comprehensive description of all actions/activities undertaken under the contract, quantitative and qualitative outcomes, problems encountered and lessons learned in the English and Mongolia languages. 

 

6.2.       Deliverables/Outputs/Results

 

The IPIU COVID-19 project shall accept each deliverable after reviewing against qualitative and quantitative indicators and confirming that it is of adequate quality and/or quantity to implement the tasks to which it relates. The Project has right to request the Consultant to revise submitted deliverables before acceptance.

 

The Consultant shall prepare and submit the following deliverables according to its contract and in the course of the assignment:

 

1.               In close coordination with the M&E specialist set up a system to objectively monitor progress in the implementation of the vaccine deployment plan and vaccine delivery distribution manual. In close coordination with the M&E specialist monitor mobile vaccination unit. Monitor the performance of vaccine deployment, identify any bottlenecks to optimal performance and suggest solutions for improvement.

2.               Develop the regular report under the scope of the COVID-19 vaccination according to World Bank Group; Prepare and deliver biweekly vaccination dashboard metrics to the World bank

3.               Develop the regular report under the scope of the COVID-19 vaccination and ESMF according to World Bank Group; Updating and revising project documents as required, as well as other sections of the Vaccine delivery and distribution manual. Any updates must be promptly submitted to the Association for a "No objection";

4.               Develop the terms of reference (TOR) for the selection of contractors for the preparation, as well as in reviewing and accepting the results of their work, specifications and statements scope of work;

5.               Establish a data exchange platform between the Ministry of Health, Ministry of Food and Light Industry and GAVS (General Authority for Veterinary Services) both for early warning systems and joint control of disease outbreaks. Organize training for health personnel with other project specialist about COVID-19 vaccines. Develop and disseminate the public messages (video, manual, booklets and others), training, and information materials aimed at improving knowledge on COVID-19 vaccination.

6.               Develop capacity for joint response to new and emerging infectious diseases. Conduct joint working group and implement multi stakeholders engaged activities on One Health.

7.               Organize Nationwide simulation together with the MOH, GAVS, MOFALI. Analyze and improve the collaboration between the MOH, GAVS, MOFALI for the prevention, detection and response to zoonotic diseases and other health events at the animal-human interface.

8.               Desk review public health law enforcement and review to address inconsistencies on One Health.

9.               Develop and update COVID-19 vaccine related webpage (admin) through establishing and operationalizing the grievance mechanism that will receive, record, resolve and respond to concerns and complaints related to post COVID-19 vaccination adverse reaction from project affected parties.   

10.           Build capacity and ensure sustainability of a grievance mechanism of the NCCD on COVID-19 vaccination, post immunization adverse reaction. Serve equity in vaccination (ex. ensuring vulnerable groups receive vaccination), boosters, and adverse reactions.

11.           After initial implementation, undertake regular site audits to monitor performance, provide feedback and correct action advice, follow up on the successful completion of corrective actions;

12.           Support the organization of training, workshops, and consultations according to the project plan in public health; Evaluation report for each training and supportive supervision conducted to be reflected in the monthly reports.

 

6.3.       Reporting and acceptance of deliverables, storage

 

The Consultant should submit two (2) hard copies of all reports and an electronic copy in an appropriate digital format. All reports, documents and other deliverables produced for and during this contract shall be the property of MOH in the Mongolian language, and in the English language if needed.

 

The quality of deliverables of consultant shall be evaluated by the Project Director (MOH). The evaluation notification will be the acceptance note of consultant signed by the project director. The project director’s acceptance note will be based on regular monitoring checklist of project coordinator of IPIU.
